With E-Z Choice, small employers can provide their employees with the peace-of-mind of income protection in case of a disability, as well as the support of a structured and caring return-to-work program.

- Benefit schedules based on percent of earnings or a flat schedule
- Elimination period - 1st day accident/8th day illness
- Benefit durations - 13 or 26 weeks

- Income replacement levels - 50%, 60%, 66 2/3% or 70%
